چکیده

A cute myocardial infarction (AMI) triggers an inflammatory reaction, which plays an important role in myocardial injury (1). Inflammatory markers such as C-reactive protein (CRP) reflect the extent of myocardial necrosis and correlate with cardiac outcomes following AMI (2–4). Hyperglycemia has proinflammatory effects, inducing the release of inflammatory cytokines (5) and is associated with increased mortality from AMI (6,7). Conversely, insulin-based therapy in patients following AMI may confer survival benefits, but the mechanism is unclear (8–11). Insulin suppresses the inflammatory response (12), but it is uncertain if the antiinflammatory effect of insulin therapy depends on glycemic status. The Hyperglycemia: Intensive Insulin Infusion In Infarction (HI-5) study is a multicentered randomized trial of insulin infusion for hyperglycemic patients with AMI. In this substudy, we examined the relationship between blood glucose levels (BGLs) and the inflammatory response (CRP levels) following AMI and evaluated the effect of insulin-based infusion therapy.

Insulin, glycemic control, and C-reactive protein during myocardial infarction.

I n recent years, much attention has been paid to the evidence that hyperglycemia during myocardial infarction enhances the risk of mortality (1). A frequent event in medicine is the “new” discovery of an old finding, and the story of hyperglycemia and acute myocardial infarction is one of these cases. An unusually high prevalence of glycosuria in nondiabetic patients who have acute myocardial ...

Comparing Effects of Continuous Insulin Infusion with or without Subcutaneous Glargine Insulin on Glycemic Control in Diabetic Patients Undergoing Coronary Artery Bypass Graft (CABG)

OBJECTIVE: Hyperglycemia is associated with increased morbidity and mortality in diabetic patients following coronary artery bypass grafting. Tight glycemic control in perioperative period can reduce these events. The goal of this study was to determine whether combination of continuous infusion and subcutaneous glargine as a basal insulin could improve glycemic control. MATERIAL AND METHODS:...

The Hyperglycemia: Intensive Insulin Infusion in Infarction (HI-5) study: a randomized controlled trial of insulin infusion therapy for myocardial infarction.

OBJECTIVE There is conflicting evidence regarding the benefit of intravenous insulin therapy on mortality following acute myocardial infarction (AMI). The goal of the current study was to determine whether improved glycemic control, achieved through an insulin/dextrose infusion with a variable rate of insulin, reduces mortality among hyperglycemic patients with AMI. RESEARCH DESIGN AND METHOD...

بررسی سطح پلاسمایی مالون دی آلدئید ، تروپونین قلبیI و پروتئین واکنشگر C در مبتلایان به بیماریهای عروق کرونر حاد

Introduction & Objective: Ischemic injury of endothelium is associated with prostaglandin synthesis and platelet adhesion and aggregation, which may be associated with the release of aldehydes such as malondialdehyde (MDA). C-reactive protein and cardiac troponin I have been proposed as diagnostic markers of acute coronary syndromes. In this study, we compared the usefulness of plasma MDA as a ...

Assessment of Periodontal Parameters and Serum Markers in Patients with Acute Myocardial Infarction

Background: Cardiovascular disease is the leading cause of death in developed countries and is a disease that has many contributing factors. Periodontal disease is one of the factors contributing to the progression of the disease. The aim of this study was to investigate the effect of periodontal disease on the incidence of acute myocardial infarction. Methods: In this case-control study, 60 su...
